Since 2020, we have been offering effective telemonitoring in accordance with the guidelines of the G-BA decision (link: https://www.g-ba.de/beschluesse/4648/(link is external)(link is external)), we have been offering effective telemonitoring for our patients with heart failure. In line with this decision, it enables us as a telemedicine center to provide comprehensive medical care for our heart failure patients using innovative technologies. By proactively monitoring key vital signs, we are able to take proactive action against potentially serious health consequences. Monitoring is carried out either via an internal cardiac device (aggregate) or external measuring devices, which can transmit vital signs relevant to heart failure to our platform (MediConnect) around the clock if required. The values are recorded there and analyzed individually for each patient using highly developed algorithms. The system issues alerts if the parameters exceed conspicuous limits, for example in the event of irregularities or elevated values, such as negative developments in the patient's condition or significant physical changes. Our team analyzes the data and then contacts the patient or the associated PBA (primary care physician) directly.

  • - NYHA II oder III und
  • - EF <40% und
  • - ICD/ CRT-P/ CRT-D- Träger oder kardiale Dekompensation mit stationärer Aufnahme innerhalb der letzten 12 Monaten und
  • - Herzinsuffizienz - leitliniengerechte Behandlung und
  • - Keine Faktoren, welche die Übertragung verhindern oder das Patienten-Selbstmanagement gefährden
